Vikash Jaiswal: Toward Better ASCVD Risk Assessment for South Asian Populations
Vikash Jaiswal, Postdoctoral Researcher at Endeavor Health, Social Media Editor at European Heart Journal Imaging Method and Practice, shared a post on LinkedIn about a recent article he and his colleagues co-authored, published in JACC: Asia, adding:
“I am excited to share our new study, ‘Comparison of ASCVD Risk Prediction Models in STEMI: Insights From a South Asian Cohort’ has been published in JACC Journals.
In this retrospective analysis of 4,975 consecutive South Asian patients presenting with first acute myocardial infarction (AMI), we evaluated the performance of five major 10-year cardiovascular risk prediction models: Framingham Risk Score (FRS), ASCVD Pooled Cohort Equations, WHO CVD risk charts, Joint British Societies’ (JBS-3), and the newer PREVENT equations.
Key Findings:
- Significant discordance in risk classification across models, highlighting limited generalizability of Western-derived tools to South Asian populations.
- PREVENT classified 19.8 percent of patients as high risk, compared with 20.2 percent using FRS, 15.0 percent using WHO charts, 11.7 percent using JBS-3, and 12.3 percent using ASCVD 2013.
- Importantly, approximately 80 percent of patients who experienced AMI were not classified as high-risk by these tools prior to their event.
These results underscore the urgent need for South Asia-specific cardiovascular risk prediction models and tailored primary prevention strategies to reduce the growing burden of premature atherosclerotic cardiovascular disease (ASCVD) in our region.
