”Unexpected Findings: Platelet Transfusions May Trigger Inflammation in Preterm Neonates
Important research from Patricia Davenport and colleagues sheds light on a concerning puzzle: why liberal platelet transfusions in preterm neonates have been linked to increased bleeding and mortality in recent trials.
The study reveals that platelet transfusions in thrombocytopenic neonates trigger inflammatory responses, including elevated RANTES (a potent chemokine) and increased neutrophil extracellular traps (NETs).
Notably, the inflammatory response correlates with platelet storage time and free mitochondrial DNA levels – pointing to specific blood banking factors that could influence outcomes.
These findings identify novel mechanisms that may explain adverse outcomes and highlight potential targets for improving transfusion safety in this vulnerable population.
Critical insights for neonatology and transfusion medicine.
