”A patient developed a pulmonary embolism.
He was started on anticoagulation.
Two days later, he developed gastrointestinal bleeding.
Anticoagulation was stopped.
Multiple specialists became involved.
One question remained:
When should anticoagulation be restarted?
It seemed like a straightforward question.
Yet, no one owned the decision.
