The management of severe aplastic anemia continues to evolve.
While horse ATG plus cyclosporine remains the backbone of frontline immunosuppressive therapy, recent studies and EHA 2026 discussions highlight the growing role of thrombopoietin receptor agonists (especially eltrombopag) in improving hematologic responses and reducing transfusion dependence.
Key trends shaping 2026 practice include:
- Earlier integration of TPO-receptor agonists into frontline therapy
- Improved risk stratification through molecular profiling and clonal evolution monitoring
- More individualized treatment selection based on age, donor availability, and genomic findings
