Low platelet count (thrombocytopenia) is a common cause of panic among patients.

In a discussion with Dr. Abhishek Shukla, Dr. Anshul Gupta, Senior Hemato-Oncologist, provided key clinical perspectives on managing this condition.

When evaluating a patient with low platelets, doctors consider three primary factors:

  • Age: Special attention is required for children (younger than 18 years) and elderly individuals (older than 60 years), where a low count might indicate underlying conditions or bone marrow issues rather than simple Immune Thrombocytopenia (ITP).
  • Clinical Appearance: Assessing whether the patient looks acutely ill or remains symptomatic-free and mobile.
  • Complete Blood Count (CBC) Profile: Checking whether other blood cell lines (WBCs and Hemoglobin) are also affected.

In ITP or post-viral cases (like Dengue), counts above 30,000 generally do not require immediate intervention, as antibody levels usually subside over time. If platelets drop below 20,000 or bleeding occurs (e.g., from the mouth, nose, or in urine), prompt evaluation and possible admission are needed.

In Dengue, repetitive platelet transfusions are often unnecessary unless clinically indicated. Regarding treatment, popular remedies like Carica papaya extract lack definitive clinical evidence.
