Anticoagulants in Pregnancy—When to Use Warfarin vs Heparin

”Anticoagulants Comparison: Warfarin vs Heparin
Warfarin and Pregnancy – Mechanical Valves (Exception)
1st trimester (Months 1–3): Warfarin is best avoided. Use LMWH/UFH.
2nd–early 3rd trimester (Months 4–8): Warfarin can be started if dose < 5 mg/day.
Late 3rd trimester (Month 9): Switch to LMWH or UFH.”
