Alireza Arzhangzadeh: Antithrombotic Strategies in Coronary Artery Aneurysm, Ectasia, and Slow Flow

“Proud to have contributed to our recently published systematic review in Clinical Cardiology.

I’m truly grateful to Prof. Hamed Bazrafshan and his team for their trust, guidance, and collaboratio n throughout this project.

Beyond the publication, three practical messages stood out:

  • CAE — Treat the thrombotic risk, but individualize the strategy.

In coronary artery ectasia, antithrombotic therapy appears beneficial compared with no therapy, but current evidence does not establish a clear advantage of OAC over DAPT.

  • Anticoagulation — Quality matters.

When VKA is used, maintaining an adequate therapeutic range (TTR of 60% or higher) may be as important as choosing anticoagulation itself.

  • CAA & CSFP — Avoid a one-size-fits-all approach

Evidence remains limited, making individualized assessment of ischemic vs. bleeding risk central to clinical decision-making.

The practical message for me: the diagnosis may guide us, but the individual patient should ultimately shape the antithrombotic strategy.”
