David Rosenblum: Can Concentrated PRP Actually Cause Scar Tissue?
David Rosenblum, CEO, President of QBazaar.com, LLC, Director of Pain Medicine, Maimonides Medical Center at AABP Pain Management, shared a post on LinkedIn:
“Due to all the interest in my rotator cuff PRP post last week, and the scar tissue questions it raised, I went back to the literature:
Can concentrated PRP actually cause scar tissue?
In animals, yes: PRP injected into healthy rabbit tissue left persistent scar at 12 weeks while saline sites stayed clean (Harris 2012), and the highest platelet dose killed tendon cells in culture (Giusti 2014).
In humans?
No study — at any concentration — has ever linked PRP to fibrosis. If you know of any studies that contradict, please let us know!
What the human data actually shows:
- Tripling concentration (1M to 3M platelets/µL) added nothing in knee OA (Dernek 2017).
- Higher concentration cut treatment failure from 15.0% to 3.3% (Boffa 2024).
- What predicts success is total dose — more than 10 billion platelets, not concentration (Berrigan 2024).
- Double the dose at the same concentration worked better; the only cost was a few days of soreness (Patel 2024).
Concentration, volume, total dose — three different numbers.
