Aabha Divya: Late Biatrial Thrombosis 31 Years After Heart Transplantation

Aabha Divya, Cardiothoracic Surgeon at East Jefferson General Hospital, Assistant Professor at Tulane University School of Medicine, Associate Editor at JACC Journals, shared a post on X about a recent article she and Niti Dalal co-authored, published in JACC: Case Reports, adding:

“‘Surgical progress does not reach backward.’

In a remarkable case published in JACC: Case Reports, Majed Tolah and colleagues describe massive biatrial thrombosis 31 years after heart transplantation.

The patient underwent transplantation using the biatrial (Lower–Shumway) technique, now largely replaced by bicaval anastomosis.

Yet many long-term survivors retain this historical anatomy, with redundant atrial tissue, extensive suture lines, and altered flow dynamics that may predispose to late thrombosis.

In our accompanying editorial, we discuss the long-term implications of surgical anatomy, anticoagulation adherence, and individualized surveillance.

As transplant survival extends into the third and fourth decades, should the original anastomotic technique influence long-term surveillance and thromboembolic risk assessment?

Congratulations to Dr. Tolah and colleagues on this important report.”
