“The stroke clock is still critical – but it is no longer the whole story.
This 2026 review highlights how acute ischemic stroke management has evolved from a primarily time-driven approach toward increasingly sophisticated tissue- and patient-selection strategies.
Tenecteplase is expanding the thrombolysis landscape.
Advanced imaging can identify selected patients who may benefit beyond traditional treatment windows.
Mechanical thrombectomy has moved well beyond the original early-window large-vessel population, with growing evidence even in patients with large ischemic cores.
At the same time, important questions remain – particularly for medium and distal vessel occlusions.
The central message is simple:
Treat fast.
Image intelligently.
Select carefully.”
