Francesca Palandri: The R-NAT Score Developed in Ruxolitinib-Treated MF Patients

“Can we predict thrombosis in myelofibrosis patients treated with ruxolitinib?
- At the SIE – Italian Society of Hematology 2026 Congress I presented the R-NAT score, developed in 1,126 ruxolitinib-treated MF patients from the RUX-MF study:
- Current models fall short in MF: IPSET-T and the conventional binary model barely separated patients by risk (C-index 0.53–0.55).
- NLR is the missing inflammatory piece: a baseline neutrophil-to-lymphocyte ratio above 3.53 (the upper limit of normal) independently doubled the risk of thrombosis (SHR 2.17), even among patients already classed as high-risk.
- A simple, validated 3-variable score: R-NAT combines age, prior thrombosis and NLR >3.53. It separates patients into three groups with 5-year thrombosis-free survival of 97.4 percent, 89.1 percent and 77.9 percent (C-index 0.72).
