Abbeha Talib: Balancing Stroke Prevention and Bleeding Risk After Successful AF Ablation

“Excited to share that our latest work has been published in Cardiology in Review:
‘Discontinuation of Oral Anticoagulants After Successful Atrial Fibrillation Ablation: A Systematic Review and Meta-Analysis.’
What we explored:
In this large-scale analysis, we examined whether oral anticoagulation (OAC) can be safely discontinued after successful catheter ablation for atrial fibrillation (AF), comparing long-term outcomes of OAC discontinuation (OFF-OAC) versus continuation (ON-OAC).
Key findings: (29 studies, 153,352 participants):
- No significant difference in thromboembolic risk with OAC discontinuation (OR: 0.91; P = 0.60)
- OFF-OAC significantly reduced major bleeding risk by approximately 66 percent (OR: 0.34; P less than 0.00001)
- No significant difference in all-cause mortality (OR: 0.67; P = 0.06)
- OAC discontinuation was associated with significantly lower AF recurrence (OR: 0.57; P = 0.0003)
Subgroup insights:
- Greater bleeding reduction with OAC discontinuation was observed in:
- Warfarin users (OR: 0.13) versus warfarin/NOAC users (OR: 0.43)
- Non-Asian cohorts (OR: 0.12) versus Asian cohorts (OR: 0.45)
- Patients switched to antiplatelet therapy (OR: 0.18) versus those not switched (OR: 0.44)
Takeaway:
Discontinuing OAC after successful AF ablation significantly reduces major bleeding without increasing thromboembolic or mortality risk in appropriately selected patients.
OAC may be safely withdrawn in low-risk patients (CHA₂DS₂-VASc less than 2) with careful monitoring, while high-risk patients warrant individualized decision-making.
