Damien Luque Paz: Improving Myeloproliferative Neoplasm Diagnosis With Non Invasive Biomarkers
Damien Luque Paz, Professor of Hematology, University Hospital Physician at CHU d’Angers, shared a post on LinkedIn about a recent article he and his colleagues co-authored, published in Leukemia, adding:
“I am very pleased to share our latest publication, now available online in Leukemia journal.
The aim was to evaluate whether a combination of non-invasive parameters could reliably distinguish essential thrombocythemia (ET), prefibrotic myelofibrosis (preMF), and overt primary myelofibrosis (PMF)
In this prospective study, we recruited newly diagnosed MPN patients in Angers and Brest Hospitals (128 patients, included in the BCB FIMBANK). We combined several non-invasive parameters: NGS sequencing, circulating CD34+ cell quantification, spleen elastography, and plasma levels of 29 cytokines and every bone marrow biopsy was centrally reviewed by three expert hematopathologists, with an independent adjudication committee integrating histology and clinicobiological data for final diagnosis classification.
The key findings are:
- The mutational landscape becomes progressively more complex from ET to preMF to PMF, with rising driver mutation allele burden
- On plasma cytokines: a distinct inflammatory profile and matrix-remodeling markers clearly set PMF apart from the other two entities.
- A two-step classification strategy (Lasso regression) combining clinical, genomic, and cytokine data identifies PMF very well (AUC 0.909), but showed more modest performance for distinguishing ET from preMF (AUC 0.762); a result consistent with the hypothesis of a pathophysiological continuum between these entities.
This work paves the way for non-invasive scores in MPNs, similar to those already used in hepatology for liver fibrosis
