ESC Guidelines – Crucial LDL Updates Highlight Risks for Extreme and Very High-Risk Patients

Christopher Cannon, Editor in Chief, Harvard Health Publishing, shared a post on LinkedIn:

“Update to the ESC Lipid guidelines! A few things I note:

A new risk category: Extreme risk with LDL goal < 40 mg/dl!
Patients with ASCVD who experience recurrent vascular events while taking
maximally tolerated statin-based therapy, or Patients with polyvascular (e.g. coronary and peripheral) arterial disease

“Very high risk” is any ASCVD – clinical or by imaging! LDL goal <55 mg/dl
Documented ASCVD, either clinical or unequivocal on imaging. Documented ASCVD includes previous ACS (MI or unstable angina),
chronic coronary syndromes, coronary revascularization (PCI, CABG, and other arterial revascularization procedures), stroke and TIA, and
peripheral arterial disease. Unequivocally documented ASCVD on imaging includes those findings that are known to be predictive of clinical
events, such as significant plaquea on coronary angiography or CT scan or on carotid or femoral ultrasound or markedly elevated CAC score
by CTb”
