”Permissive Hypertension For Ischemic Stroke
In 2026, permissive hypertension remains a standard temporary management strategy for acute ischemic stroke.
It involves intentionally allowing a patient’s blood pressure (BP) to remain elevated to ensure adequate blood flow to the ischemic penumbra—the salvageable brain tissue surrounding the core area of the stroke.
It’s purposely done to maximize cerebral perfusion and promote collateral circulation when the brain’s natural autoregulation of blood flow is impaired.
It’s typically maintained for the first 24hrs to 48hrs following stroke onset.
It’s Primarily candidates are patients with acute ischemic stroke who are not candidates for immediate reperfusion therapies (like “clot-busting” drugs) or those whose BP is high but below dangerous thresholds (<220/120 mmHg)”
