”Some cases become memorable not only because of clinical complexity, but because of the story behind them.
I recently managed a young postpartum patient admitted with severe sepsis complicated by disseminated intravascular coagulation and acute kidney injury.
What made this case particularly interesting was the background.
At a young age, the patient was newly diagnosed with Factor V Leiden mutation and was informed that future pregnancy would carry significant risks.
After careful consideration, she decided to proceed with ovarian stimulation, became pregnant with twins, and was receiving therapeutic-dose enoxaparin throughout pregnancy.
Sadly, she experienced intrauterine fetal demise at 8 months, followed by rapid clinical deterioration and multi-system involvement.
The case required continuous reassessment, careful balancing between thrombosis and bleeding, and close multidisciplinary collaboration between critical care, obstetrics, nephrology, and hematology.
Step by step, with timely supportive care and coordinated decision-making, the patient improved and was successfully discharged.
This experience reinforced how complex medical decisions, patient choices, and physiology intersect in critical care — and how individualized management can make a real difference.”
