Dirk Sibbing: Potent P2Y12 Inhibition in Patients With Atrial Fibrillation and Acute Coronary Syndrome

“EPIDAURUS Trial – presented at ESC 2026 and published simultaneously in nature medicine
DOAC plus potent P2Y12 inhibition vs. DOAC plus clopidogrel and in-hospital aspirin in ACS patients (n=602) with atrial fibrillation undergoing PCI
Treatment with a potent P2Y12 inhibitor was associated with higher bleeding rates without a clear reduction in the risk of ischemic complications
Key take: More potent platelet inhibition does not necessarily result in less ischemic events
Clinical scenarios differ in terms of assets and drawbacks of potent platelet inhibition with prasugrel or ticagrelor
