Krisstina Gowin: Could the Gut Microbiome Be the Missing Link in Hematology Outcomes?
Krisstina Gowin, Clinical Associate Professor at City of Hope, shared a post on LinkedIn:
“Could the gut microbiome be one of the most important—and most overlooked—factors influencing outcomes in hematology?
In our latest ASH Clinical News guest commentary, Dr. Urvi Shah and I explore the growing evidence that the gut microbiome plays a critical role in allogeneic stem cell transplantation and CAR T-cell therapy.
Emerging data suggest that microbial diversity, short-chain fatty acids, bile acid metabolism, dietary patterns, and even antibiotic exposure can influence treatment response, toxicity, graft-versus-host disease, and survival.
As hematologists, we meticulously optimize conditioning regimens, chemotherapy dosing, and supportive care. It may be time to bring microbiome health into that same conversation.
Key takeaways:
- Microbial diversity is associated with improved transplant outcomes.
- Fiber-rich, plant-forward dietary patterns support beneficial microbiome composition and metabolite production.
- Broad-spectrum anti-anaerobic antibiotics may adversely affect outcomes in both alloHCT and CAR T-cell therapy.
- The microbiome is emerging as a powerful biomarker and potential therapeutic target in hematology.
The future of hematology may not only depend on understanding the cancer and the immune system—but also the trillions of microbes that help shape both.
Thank you to my co-authors and collaborators, Dr. Urvi Shah, Dr. Jenny Paredes, and Dr. Marcel van den Brink, for advancing this exciting field.
