“Platelet Satellitism — when platelets form rosettes around neutrophils, creating a striking microscopic appearance.
An EDTA-dependent in vitro phenomenon, platelet satellitism occurs due to autoantibodies that promote platelet adherence to neutrophil Fc receptors in the presence of EDTA.
Although clinically insignificant, it can lead to pseudothrombocytopenia on automated analyzers, potentially triggering unnecessary investigations or treatment if unrecognized.
Key takeaway: Always correlate an unexpected low platelet count with peripheral smear findings and consider repeating the CBC in a citrate or heparin tube.
A reminder that not all thrombocytopenia is real – sometimes the platelets just prefer company!”
